Online services are currently used to access your personal medical records to view medication, allergies, adverse reactions and immunisations. You can also book face to face GP appointments and request repeat medication.
If you do not have an online account already please download and print the form below or collect a form from reception. You will need to attend in person with one form of photo ID for this service. Once you have handed in your form, a set of codes will be generated up to 1 week later which can be collected from the surgery, you will need to provide your ID to collect this. Access to online services is not available to anyone under the age of 16.
Although the main service provider recommended by the practice for this is Patient Access or the NHS App, other service providers are available and a full list can be obtained when collecting your online access pin codes from reception.
