Your social prescribing link worker can talk to you about a problem you have or a change you would like to make. Your link worker will not only give advice and encouragement, they will also connect you with suitable local services to support you.

Erica Jacobs is our Social Prescriber via the Hayewick PCN (Primary Care Network) she is available to help with the following:

You can book an appointment with Erica via reception where they will complete a short referral form, you do not need to see a GP to be referred.
